The Florida Department of Health in Highlands County is committed to preventing diabetes for those at-risk, and to providing quality public health education programs focused on decreasing risk for and managing the consequences of this disease. Closing the GAP is a grant-funded program that provides public diabetes prevention and self-management classes, and works to reduce racial and ethnic health disparities in Highlands County.

Projects funded through this program support both public and private entities by:

  • Fostering partnerships between local governments, community groups, and private sector health care organizations
  • Helping communities address their most pressing health needs theorugh targeted health screenings, education, and awareness programs
  • Helping communities better understand the natrue of health disparities among ethnic and racial groups
